Inclusion Criteria

• A Mayo Clinic patient within the Pain Division piloting PGx testing with the Center for Individualized Medicine, seeking clinical information from emerging PGx testing technologies available in CLIA/CAP laboratories
• Patient age 18 or above
• Patient must reside in a county within a 100 mile radius of Rochester, MN
• Patient must be taking a medication of interest (classes of medications include TCA, SNRI, opioid, or benzodiazepine)
• Patients must understand and provide written informed consent and HIPAA authorization prior to initiation of any study-specific procedures
• Patient is willing to engage in a medication assessment as part of their clinical visit preparation (when needed)

Exclusion Criteria

• Patient with uncontrolled concurrent illness including psychiatric illness, or situations that would limit compliance with the study requirements or the ability to willingly give written informed consent
