跛行
跛行是指步行时腿部疼痛或使用手臂时手臂疼痛。疼痛是因流向腿部或手臂的血流过少而引起的。跛行通常是外周动脉疾病的一种症状。患外周动脉疾病后,向手臂或腿部(通常是腿部)供应血液的动脉变窄。变窄通常是由于动脉壁上的脂肪沉积(斑块)越来越多。
外周动脉疾病是一种因动脉狭窄导致上下肢血流量减少的常见状况。
外周动脉疾病(PAD)患者上下肢供血不足(常见于下肢),无法满足身体需求。这可能会引起行走时腿痛(跛行)等症状。
外周动脉疾病通常是脂肪沉积物在动脉中积聚(动脉粥样硬化)的体征。动脉粥样硬化会引发动脉狭窄,从而导致下肢(有时伴随上肢)的血流量减少。
外周动脉疾病的治疗方案包括运动、健康饮食以及不抽烟或不使用烟草。
症状
许多外周动脉疾病患者只有轻微症状或没有症状。但部分患者在行走时会感到腿痛(跛行)。
跛行症状包括肌肉疼痛或腿部或手臂抽筋,通常在运动中开始并在休息时结束。疼痛最常见于小腿。疼痛程度从轻微到严重不等。严重腿痛可能导致难以行走或难以从事其他类型的身体活动。
其他外周动脉疾病的症状可能包括:
- 小腿或足部冰凉,尤其是与另一侧相比
- 腿部麻木或无力
- 腿部或足部没有脉搏或脉搏微弱
- 在行走或爬楼等特定活动后,一侧或两侧臀部、大腿或小腿肌肉出现痛性痉挛
- 腿部皮肤产生光泽
- 腿部皮肤颜色改变
- 脚趾甲生长放缓
- 脚趾、足部或腿部出现无法愈合的伤口
- 手臂活动时疼痛,例如编织、书写或进行其他手工活动时出现疼痛和痛性痉挛
- 勃起功能障碍
- 腿部体毛脱落或生长趋缓
如果外周动脉疾病加重,在休息或躺下时也可能产生疼痛。疼痛可能打断睡眠。将腿悬垂在床边或走动可以暂时缓解疼痛。

如果血液中的胆固醇过多,胆固醇和其他物质可能形成沉积物(称为斑块)。斑块会导致动脉变窄或阻塞。如果斑块破裂,就会形成血凝块。斑块和血凝块会使通过动脉的血流减少。
外周动脉疾病通常由附着在动脉血管壁上含脂肪和胆固醇的沉淀物(斑块)所导致。该过程称为动脉粥样硬化,其会导致通过动脉的血流量减少。
动脉粥样硬化会累及全身的动脉血管。如果向四肢供血的动脉出现动脉粥样硬化,则会引起外周动脉疾病。
不太常见的外周动脉疾病原因包括:
风险因素
抽烟或患糖尿病会大大增加患外周动脉疾病的风险。其他导致外周动脉疾病风险增大的因素包括:
- 外周动脉疾病、心脏病或卒中家族病史
- 高血压
- 高胆固醇
- 氨基酸含量过高(称为同型半胱氨酸),可增加患冠状动脉疾病的风险
- 年龄增长,特别是 65 岁以上(如果存在患动脉粥样硬化的风险因素,则为 50 岁以上)
- 肥胖(体重指数大于 30)
并发症
动脉粥样硬化引起的外周动脉疾病并发症包括:
- 严重肢体缺血。在这种情况下,损伤或感染会导致组织死亡。症状包括肢体开放性疮不愈合。治疗可能包括患肢截肢术。
- 卒中和心脏病发作。动脉中的斑块积聚也可能影响心脏和脑中的血管。
预防
预防外周动脉疾病引起的腿痛的最佳方法是保持健康的生活方式。健康的生活方式是指:
- 不要抽烟。
- 控制血糖。
- 食用饱和脂肪含量低的食物。
- 定期运动,但要向医务人员咨询最适合您的运动类型和强度。
- 保持健康的体重。
- 管理血压和胆固醇。
