SUMMARY

Hemang Yadav, M.B.B.S., is a clinical and translational researcher who focuses on improving understanding of acute respiratory failure (ARF) and acute respiratory distress syndrome (ARDS) in the intensive care unit (ICU). Dr. Yadav's primary research focus is on improving understanding of the mechanisms of respiratory failure after hematopoietic stem cell transplantation (HCT) with the ultimate goal of developing novel ARDS prevention strategies focused on immunocompromised patients. Dr. Yadav is also studying ARF due to infections, with ongoing studies in hospitalized patients with influenza and COVID-19.

Focus areas

  • Respiratory failure in patients after hematopoietic stem cell transplantation. Dr. Yadav is focused on furthering the understanding of why patients who undergo HCT develop respiratory failure after transplantation. His epidemiological work has highlighted the frequency and devastating impact of ARDS on patients who have undergone stem cell transplantation. Dr. Yadav is currently investigating how novel and potentially modifiable risk factors affect patients' chances of developing ARDS after stem cell transplantation. This understanding can facilitate targeted mechanistic studies and enrolling the highest risk patients into ARDS prevention clinical trials.
  • Biomarkers and metabolomics in ARDS. Dr. Yadav has studied the pathophysiology and immunology of postoperative ARDS by analyzing the temporal profile of key biomarkers in the perioperative period. He has studied the integration of biomarkers into clinical scoring systems to better identify those at risk of developing ARDS. Ongoing research projects are exploring the role of biomarkers in post-HCT ARDS. In addition, Dr. Yadav has partnered with the Mayo Clinic Regional Comprehensive Metabolomics Resource Core to investigate metabolomics in ARDS development.
  • Utilizing precision medicine methodologies to phenotype ARDS. Dr. Yadav's research team seeks to integrate clinical, biomarker and metabolomic data to explore phenotypes of post-HCT ARDS. Utilizing machine learning methodologies alongside the biomarker and metabolomics work outlined above, Dr. Yadav is investigating whether subtypes of post-HCT ARDS can be identified. This ultimately seeks to move away from a 'one size fits all' approach to such patients, and towards more individualized care of post-HCT ARDS patients.
  • Individualizing steroid therapy in COVID-19 pneumonia. Corticosteroids have been shown to improve survival in severely ill patients with COVID-19 pneumonia. Dr. Yadav is the principle investigator for the Steroid Dosing by bioMARker Guided Titration in Critically Ill Patients With Pneumonia trial (ClinicalTrials.gov Identifier: NCT03852537). This trial developed and utilizes a novel biomarker-titrated individualized approach to adjuvant steroid dosing in hospitalized patients with COVID-19 pneumonia.

Significance to patient care

Acute respiratory distress syndrome (ARDS) is a frequent complication after HCT, dramatically influencing patient survival and quality of life. To date, effective treatments for established ARDS are few, and the impact of ARDS for individual patients can be devastating. Dr. Yadav's hope is that a better understanding of the risk factors, immunology and pathophysiology of ARDS can help identify those patients at increased risk of developing ARDS in the post-transplant period. His long-term research focus is to then develop targeted preventive strategies for high-risk individuals in order to prevent ARDS before it occurs.
