    The objectives of the study are to evaluate patient satisfaction as well as perceived benefit and value from their visit with each type of provider (i.e., physician, psychologist, physical therapist) and with the team-based model as a whole and to assess the likelihood and comfort level of seeking treatment with each type of provider if they had not been part of a team or available at a single site.
