A Study to Improve Diabetes Management Among Adults Experiencing Severe Hypoglycemia

About this study

The purpose of this study is assess the feasibility, effectiveness, and acceptability of Diabetes-REM (Rescue, Engagement, and Management), a comprehensive community paramedic (CP) program to improve diabetes self-management among adults in Southeast Minnesota (SEMN) treated for servere hypoglycemia by the Mayo Clinic Ambulance Services (MCAS).

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria - Aim 1:

  • Adults 18 years of age or older.
  • Mayo Clinic Ambulance Service (MCAS) encounter for level 2 hypoglycemia (blood glucose < 54 mg/dL) or level 3 hypoglycemia (any glucose level associated with altered mental status and/or physical functioning assistance from another person for recovery) treated with glucose, dextrose (D10W, D25W, D50W), or glucagon.
  • Type 1 or type 2 diabetes.
  • Paneled to a Mayo Clinic or Mayo Clinic Health System practice.
  • Able to provide informed consent.
  • Community-dwelling in either Freeborn, Mower, Olmsted, Steele, or Wabasha county.

Exclusion Criteria:

  • Under 18 years of age.
  • Cognitive impairment precluding informed consent.
  • Lack of conversational English skills.
  • Residency in a long-term care facility.
  • Enrolled in hospice.
  • Enrolled in a care coordination or disease management program.
  • Advanced or terminal illness.

Inclusion Criteria - Aim 2:

  • Participants who were randomized to and completed the D-REM intervention will be purposefully sampled to represent a priori defined differing demographic, clinical, and geographic sub-groups.

Exclusion Criteria:

  • Under 18 years of age.
