About this study

The purpose of this study is to:

  1. To identify the patient-centric factors of greatest concern to patients with HF.
  2. To determine patient characteristics associated with patient-centric factors such as social support and self-management.   

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:  

  • An adult patient who is 30 years of age or older.
  • The cover letter included with the survey packet meets the requirements of an oral consent. 
  • Patient must be able to provide this consent by reading the cover letter and completing the survey.

Exclusion Criteria: 

  • Any patient under 30 years of age.
