About this study

The purpose of this study is to evaluate the associations between social support and hospitalizations and assess whether social support interacts with coexisting conditions to predict hospitalizations. This study will also evaluate the associations between patient self-management and hospitalizations and assess whether patient self-management mediates the association between coexisting conditions and hospitalizations.

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Male or female, ≥ 18 years of age.
  • Resident of the 11-county region of southeastern MN (Dodge, Fillmore, Freeborn, Goodhue, Houston, Mower, Olmsted, Rice, Steele, Wabasha, and Winona Counties)\.
  • Patient of Mayo Clinic, Mayo Clinic Health System or Olmsted Medical Center. 

Exclusion Criteria:

  • < 18 years of age.
