About this study

The objective of this project is to capture stories as well as develop and evaluate communication tools for the clinical encounter that encourage patients to introduce information from their lives that they believe to be relevant to their lives and ability to enact self-care.

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Clinician:
    • Clinician at participating clinic/hospital
  • Patient:
    • Adults ≥ 18 years
    • Appointment with a clinician

Exclusion Criteria:

  • Clinician:
    • None
  • Patient:
    • Major barriers to participate in decision making or to providing informed consent (i.e. dementia, severe hearing or visual impairment)
